Player Waiver and Release of Liability

The following terms and conditions related to player participation in training sessions, programs, or events (“Activity”) offered by Felix Training Academy (“Academy”):
 

1. Assumption of Risk

I understand that participating in physical activities, including but not limited to sports training, exercises, and related activities, carries inherent risks of injury, illness, or other potential harm. I acknowledge that these risks may include, but are not limited to, physical injury, strain, broken bones, sprains, and heart-related issues. I voluntarily choose to assume all such risks, whether known or unknown, associated with participating in these activities.
 

2. Medical Conditions

I represent that I am in good health and capable of safely participating in the Activity. I acknowledge that it is my responsibility to seek medical advice if I have any medical conditions, concerns, or special health considerations that may affect my ability to safely participate in the Activity. I agree to immediately inform the Academy of any such concerns before beginning or continuing participation.
 

3. Release of Liability

I, on behalf of myself, my heirs, assigns, and legal representatives, do hereby release, discharge, and hold harmless Felix Training Academy, its owners, employees, instructors, agents, and affiliates, from any and all claims, demands, actions, or causes of action for personal injury, death, or property damage that may arise from my participation in the Activity, whether caused by the negligence of the Academy or otherwise.
 

4. Indemnification

I agree to indemnify, defend, and hold harmless Felix Training Academy and its affiliates from any loss, liability, or costs, including legal fees, arising out of my participation in the Activity, including claims made by third parties.
 

5. Photographic and Media Release

I grant permission to Felix Training Academy to take photographs, video recordings, or other media of my participation in the Activity, and I understand that such media may be used for promotional, educational, or other purposes related to the Academy’s operations. I waive any rights to compensation or ownership of such media.
 

6. Rules and Regulations

I agree to abide by all rules, regulations, and guidelines set forth by Felix Training Academy for the safe and effective participation in the Activity. I understand that failure to comply with these rules may result in immediate suspension or termination of my participation, without refund.
 

7. Cancellation and Refunds

I acknowledge that Felix Training Academy has specific policies related to cancellations, rescheduling, and refunds. I agree to abide by these policies as outlined in the Academy's booking policies.
 

8. Severability

If any part of this waiver is found to be invalid or unenforceable by a court of law, the remainder of the waiver will remain in full force and effect.
 

9. Governing Law

This waiver shall be governed by and construed in accordance with the laws of the state or jurisdiction in which Felix Training Academy is based, and any disputes arising from this waiver will be resolved in the courts of that jurisdiction.

FTA Monthly Membership Policy: Training Kit Requirement

1. Overview:
All players are required to wear the official FTA (Felix Training Academy) training kit during every training session. This policy promotes professionalism, team spirit, and consistency.

2. Training Kit Purchase Requirement:

Families must purchase the official FTA training kit for each registered player upon enrollment in the monthly membership program.

The kit includes two (2) training shirts and one (1) training short. Additional items are optional.

3. Purchasing the Training Kit:

Kits can be ordered through the FTA online store, with a link provided after registration.

Select sizes and quantities during purchase to ensure proper fit and availability.

Kits must be purchased within two weeks of enrollment or membership renewal.

​

4. Wearing the Training Kit:

The FTA training kit is mandatory for all sessions unless otherwise specified.

Players without the full kit may be asked to observe rather than participate.

Club-branded apparel is strictly prohibited.

​

5. Maintenance and Replacement:

Families are responsible for keeping the kit clean and in good condition.

Replacement items are available through the FTA store as needed.

6. Exceptions and Special Circumstances:

Kits take 5-7 business days to arrive and are shipped to the address provided during purchase.

If a kit is delayed or unavailable, notify FTA staff in advance.

While awaiting the kit, players may wear red, black, white, or grey tops with black shorts. Club logos are not allowed.

7. Enforcement and Compliance:

Compliance is mandatory under the FTA membership agreement.

Non-compliance may result in warnings, session suspension, or other disciplinary actions.

Felix Training Academy: Booking Policy

At Felix Training Academy, we are dedicated to providing exceptional training. To ensure a smooth experience, please review our booking policy below.

1. Booking Process
Online Bookings: All training sessions must be booked via our website or training platform.
Confirmation: Your session is confirmed once you receive an email or SMS confirmation.
Payment: Full payment is required at the time of booking to secure your spot.


2. Booking Lead Time
Advance Booking: Book at least 7 days in advance to ensure availability.
Last-Minute Bookings: We accept last-minute bookings, but availability is subject to instructor schedules.


3. Rescheduling and Cancellations
Rescheduling: You can reschedule your session with 48 hours' notice. Alternative dates will be offered based on availability.
Cancellation Policy:
More than 48 hours' notice: Full refund, minus transaction fees.
Less than 48 hours' notice: 50% refund.
No Show: No refund for missed sessions without prior notice.


4. Late Arrival
Late Policy: If you arrive late, your session will end at the scheduled time, and the full fee will apply. No extension will be provided.


5. Transfer of Session
Sessions are non-transferable between clients but may be transferred to another individual with prior approval.
 

6. Special Requests
Custom Training: Contact us at least 7 days in advance for tailored training needs.
Accessibility: Please inform us of any special accommodations or accessibility requirements when booking.
 

7. Payment Methods
Accepted Payments: We accept credit/debit cards and bank transfers.
Invoicing: Corporate bookings may request invoices. Payment terms are 30 days from the invoice date.
 

8. Refunds and Disputes
Refunds are processed according to the cancellation policy.
Disputes should be directed to support via email. We will work to resolve issues promptly.
 

9. Health and Safety
Medical Conditions: Ensure you are physically fit to participate. Consult with a doctor if needed.
COVID-19 Protocols: By booking, you agree to comply with all health and safety guidelines in effect at the time.
 

10. Policy Updates
Felix Training Academy reserves the right to update this policy. Any changes will be communicated via email or our website.
